The Health Information Management department at PIW may be reached by fax at 202-595-0961 between the hours of 8 am and 4:30 pm, Monday through Friday. Requests can also be sent via email to [email protected].
Records Request
Release of patient information requires written authorization to disclose health information that meets the privacy requirements of the District of Columbia and federal regulations. For patients aged 14 through 17, both the patient and a legal guardian must authorize the disclosure of protected health information. Copies of medical records are available upon written request with a valid patient authorization.

A Model Attestation Form is required if the medical records requestor belongs to any of the below groups:
- Health Oversight Agencies
- Judicial Administrative Proceedings
- Law Enforcement
- Coroners or Medical Examiners
